If you are a new client to Blue Ridge Telehealth (never been seen before) or if it has been over a year since your last appointment and would like to get an appointment with one of our counselors, you can click the button below to complete our intake form OR call us at 540-772-8043.

Instructions & Notes

We are pleased that you have taken this first step towards finding a mental health provider who works for you. To set up your first appointment, feel free to call our Roanoke office at 540-772-8043, or complete the New Client Form.

You will be emailed an invitation to your portal. On the portal you will need to:

  1. Add a credit card on file for payments, if you choose. Visa, Mastercard and Care Credit Mastercards are accepted. 
  2. Verify and update your personal information.
  3. Verify and update your insurance information, including uploading images of the front and back of your card.
  4. Complete all of the tasks (mostly forms) that have been assigned to you or your child.

Everything is quick, easy, secure, and confidential to complete online. The portal and forms are designed to work on any screen from phone to desktop computer.

It is recommended you use Google Chrome as your browser for the best experience.

We bill most health insurance companies. Our billing experts will check your plan prior to your first session.

All portal tasks (pre-appointment paperwork) must be completed before your first appointment.

Psychotherapy Fees:

The fee for an initial psychotherapy session is $135. Each successive session is $125. Reduction in fees is considered in some cases. Co-pays, deductibles, and fees are due at the time of service. For your convenience, fees can be paid by credit card (including Care Credit Mastercard), check, or cash in our offices.
